Medical Record Reviews

Comprehensive review of military and civilian medical records to identify:

• Service-connected conditions

• Chronic illnesses

• Secondary medical conditions

• Progression or worsening of symptoms

• Functional limitations impacting daily living

Our reviews are detailed, organized, and focused on identifying medical evidence that may support VA disability claims or increase requests.

​Important Notice

At Your Place Healthcare does not guarantee VA disability approval, rating increases, or claim outcomes. All evaluations and medical opinions are based on clinical findings, available medical records, reported history, and professional medical judgment. Final disability determinations are made solely by the Department of Veterans Affairs.

 

Fee Schedule

• Initial Consultation: $50
  A one-on-one consultation to discuss concerns, goals and potential areas of assistance.

• Medical Records Review: $300
  Includes a comprehensive review of up to 500 pages.

• Additional Medical Records: $100
  Charged for each additional 200 pages beyond the initial 500 pages.

• Medical Packet Preparation: based on services requested
  Includes preparation of a comprehensive medical packet addressing up to three areas or diagnoses.

 

Following the medical-record review, a 50% deposit of the medical packet fee will be required before packet preparation begins. When a veteran proceeds with the packet service, 50% of the medical-record review fee will be credited toward the final medical packet balance.
