Veteran Services Offered

Medical Record Reviews

Comprehensive review of military and civilian medical records to identify:

  • Service-connected conditions

  • Chronic illnesses

  • Secondary medical conditions

  • Progression or worsening of symptoms

  • Functional limitations impacting daily living

Our reviews are detailed, organized, and focused on identifying medical evidence that may support VA disability claims or increase requests.

Nexus Letter Evaluations

Independent Medical Opinion (IMO) / Nexus Letters

A Nexus Letter is a professional medical opinion used to help establish a connection between a veteran’s current medical condition and their military service.

At Your Place Healthcare provides thorough nexus evaluations that may include:

  • Review of service treatment records

  • Review of VA medical records

  • Review of private medical records

  • Evaluation of current symptoms and diagnoses

  • Medical literature review when appropriate

  • Professional medical opinion regarding service connection

Nexus opinions are completed using evidence-based clinical reasoning and appropriate medical documentation standards.

Disability Benefits Questionnaires

Comprehensive DBQ Evaluations

At Your Place Healthcare assists veterans by conducting detailed assessments that support completion of Disability Benefits Questionnaires (DBQs) used by the Department of Veterans Affairs.

DBQ evaluations are designed to document:

  • Severity of symptoms

  • Functional limitations

  • Occupational impact

  • Frequency and duration of symptoms

  • Disease progression

  • Range of motion and physical findings when applicable

Our goal is to ensure evaluations are accurate, detailed, and reflective of the veteran’s current health condition.

Secondary Condition Evaluations

Many veterans develop secondary conditions related to existing service-connected disabilities. We assist with identifying and evaluating conditions that may be linked to primary diagnoses.

Examples may include:

  • Depression related to chronic pain

  • Sleep apnea secondary to PTSD

  • Radiculopathy related to spinal injuries

  • Anxiety associated with chronic illness

  • Migraines secondary to cervical conditions

Functional Impact Assessments

We provide evaluations focused on how medical conditions affect:

  • Employment

  • Daily functioning

  • Mobility

  • Sleep

  • Concentration

  • Social interaction

  • Quality of life

These assessments help provide a clearer understanding of the overall impact of a veteran’s condition.

Important Notice

At Your Place Healthcare does not guarantee VA disability approval, rating increases, or claim outcomes. All evaluations and medical opinions are based on clinical findings, available medical records, reported history, and professional medical judgment.
