MR. BASS MEDICAL PACKET
FEE SCHEDULE

This example is provided for illustrative purposes only to demonstrate how the process works and how fees may be calculated. Actual fees may vary based on the number of records reviewed and the services requested.

Mr. Bass  650 Pages of Medical Record

 Client completes an initial consultation, submits 650 pages of medical records, and proceeds with a Medical Packet Preparation at the rate of $1,800.

Medical records are thoroughly reviewed before determining whether to move forward with Medical Packet Preparation. The strongest opportunities for increase are identified during review.

Step 3

Confirmation of Service

Once the medical records review is complete and the client elects to move forward:

Medical Packet Preparation Fee: $1,800 

50% deposit to move forward with Medical Packet Preparation

Step 4

Final Payment

The total medical records review fee in this example is $400

($300 + $100 additional-page fee).

50% of the Medical Records Review cost is applied as a credit toward the final payment:

$400 × 50% = $200 Credit

Remaining 50% of Medical Packet Fee: $900
Less Records Review Credit: −$200
