At Your Place Healthcare (AYPH) provides healthcare through a combination of occupational health services, office-based care, employer-site services, community health initiatives, and the Wellness Within Reach Mobile Health Program.

AYPH operates as a cross-trained, team-based practice. Employees may assist in multiple service areas based on organizational needs, training, competency, and applicable scope-of-practice requirements.

​

AYPH's normal business schedule is generally 8:30 AM–5:00 PM; however, mobile clinics, employer services, community events, school programs, and other onsite activities may require occasional early-morning, evening, or weekend work.

​

All employees must be willing to complete AYPH-required training and qualification for federal and non-federal drug and alcohol testing responsibilities appropriate to their assigned role, including applicable DOT and non-DOT procedures, chain-of-custody requirements, specimen integrity, and collection procedures. Cross-training does not authorize an employee to perform services outside of his or her education, demonstrated competency, licensure, certification, or legal scope of practice.

Mobile Program Coordinator

Employment Type: Full-Time
Reports To: Executive Director / Clinical Director
Work Setting: Office, Mobile Medical Unit, Employer Sites, and Community Locations: Marion County and surrounding areas

Position Summary

The Mobile Program Coordinator/Operator supports the daily operations of At Your Place Healthcare's Occupational Health Services and Wellness Within Reach Mobile Health Program. This position coordinates mobile clinic schedules, employer services, community locations, program logistics, staff workflow, and mobile-unit readiness.

Community Health Worker

Employment Type: Full-Time
Reports To: Mobile Program Coordinator / Clinical Leadership
Work Setting: Office, Community, Mobile Medical Unit, Employer Sites, and Partner Locations

Position Summary

The Community Health Worker serves as a connection between AYPH, patients, healthcare providers, community organizations, employers, and social-service resources. The position supports both Wellness Within Reach and AYPH Occupational Health Services through outreach, patient navigation, health education, enrollment assistance, follow-up, and community engagement.

License Practical Nurse

Employment Type: Part-Time or Full-Time
Reports To: Nurse Practitioner / Clinical Director
Work Setting: Office, Mobile Medical Unit, Employer Sites, Schools, and Community Locations

 

Position Summary

The Licensed Practical Nurse provides nursing and clinical support for AYPH Occupational Health Services and the Wellness Within Reach Mobile Health Program. The LPN works under appropriate provider direction while supporting office-based, mobile, employer-site, preventive-health, and community services.

Medical Assistant/
Phlebotomy Technician

Position Summary

The Medical Assistant/Phlebotomy Technician provides clinical support for AYPH Occupational Health Services and the Wellness Within Reach Mobile Health Program. The position assists with patient preparation, phlebotomy, diagnostic testing, occupational-health services, chronic-disease screening, and mobile-clinic operations.

Medical Office Assistant

Position Summary

The Medical Assistant/Phlebotomy Technician provides direct clinical support across AYPH's occupational-health services and Wellness Within Reach Mobile Health Program.

This position supports high-volume screenings, laboratory services, specimen collection, occupational-health testing, patient preparation, and mobile-clinic operations while allowing the licensed provider to focus on assessment, diagnosis, treatment, and clinical decision-making.
